Early March 2024, Integrated Behavioral Health will open a practice at Wellness Way, the new cutting edge medical facility for CCP and CDPHP located at 6 Wellness Way, Latham. Wellness Way will be location for CCP patients of CCP practices that do not have a behavioral health consultants embedded in their office. If you already have a behavioral health consultant embedded in your CCP doctor’s office, nothing changes; you will continue to see your BHC in that location.

Our mind and body are connected, and your care should reflect that. At Integrated Behavioral Health (IBH), our team offers patients services for behavioral changes and overall behavioral health including mental health, substance abuse, and lifestyle medicine.

Our behavioral health consultants are integrated into the majority of our primary care practices at CCP, and our services are also available to the community without a referral. Integrated Behavioral Health is available for patients of all ages who are in need of behavioral health or psychiatry services.

We have an option for telemedicine services if you cannot make it into the office. Our licensed behavioral health consultants and board-certified psychiatrists can speak with you using your smartphone, tablet, or computer while you remain safely at home, when appropriate. If you are interested in speaking with someone from our team, simply call our behavior health schedulers, send in a request by clicking the “Become Our Patient” button on this page, or tell your Community Care Physicians provider that you would like to be seen. Within a short time, you will be connected to one of our licensed counselors. We welcome new patients, including patients who are not already a part of Community Care Physicians.
