EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Tamara John

Biography

Tamara John is a Mental Health Counselor and works as a Behavioral Health Consultant (BHC) in Integrated Behavioral Health. BHCs at Community Care Physicians are embedded within our practices working side by side with your doctor and healthcare team in delivering you the best “whole person” healthcare possible.

Tamara is patient-centered. As a clinician, she works with her patient’s best interests at heart and makes sure they are part of their treatment plan. When caring for patients, Tamara believes providing patients with all the necessary resources is most important.

Tamara’s favorite part of working as a BHC is open communication. She enjoys being able to talk through things and listen to the whole issue. Tamara takes the time to understand where her patients are coming from.

Outside of work, Tamara enjoys reading, traveling, and listening to music.
