Community Care Physicians is focused on the physician-led model of medicine.
CCP is a physician system. Decisions are made by physicians. As such, we are guided by those who practice in our offices every day. The daily practice of medicine is focused on creating an optimal experience for the patient and practitioner.
CCP embraces the independent, local practice of medicine and provides the framework and support that allows doctors to focus on what they do best – practice medicine. Compensations are competitive and decentralized based on performance. We have providers who have practiced at CCP throughout most of their careers. Attrition is usually due to retirement. Our organization has been highly successful over the past 35+ years. We encourage practitioners to participate in decisions, so we can remain focused on how our goals and objectives impact our team and our patients.
Centralized Support Services
Our organization has a clinical arm, Community Care Physicians, and a non-clinical arm, our Management Services Organization (MSO) called CCP TASS, Inc. (TASS), in partnership with CDPHP. TASS provides all non-clinical functions so our clinicians can focus on patient care.
The healthcare landscape is ever changing, and the requirements placed upon practitioners can oftentimes seem like a burden. All CCP practices across the Capital Region receive centralized technical, administrative, and business support services through the MSO, TASS. This distinctive structure allows our physicians and advanced practitioners to focus solely on the practice of medicine, and our practitioners find special value in these services.
Our unique business model is attractive to practitioners and practices who find business functions like billing, compliance, and IT overwhelming. With the business operations centralized, our providers don’t need to outsource these responsibilities or take time away from patients to manage these functions themselves.
TASS, located in our Latham, NY campus at the Capital Region Health Park, is directed by our experienced health management team and includes:

Physical Wellness
CCP has a high engagement and affordable wellness program guided by physicians. Almost 35% of our workforce is participating in the program since its inception.
Commitment to Innovation
CCP’s medical practices belong to a single integrated health system, sharing a single practice management and electronic health records platform, Allscripts. Our practices and patients are connected utilizing a secure patient portal called FollowMyHealth™. CCP is a founding member of HIXNY, the Regional Health Information Organization, and shares clinical data electronically with hospitals, health plans and other physicians throughout the region. Additionally, we have developed and integrated technologies into our work-flows, including a telemedicine platform that allows for remote virtual video visits with our patients, and also TeleCare, a hands-free virtual scribing system that allows practitioners to work more efficiently and spend less time after hours with patient charts.
Financial wellness
- CCP formed its own Risk Retention Group (RRG), or captive insurance company, to cover the professional liability risks of our physicians and other practitioners. All practitioners are eligible to become shareholders in the RRG.
- CCP offers a robust 401(k) for employees, which allows for post-tax contributions. CCP puts in a minimum of 3%. This is not a matching system, so you do not have to contribute to get a contribution.
- CCP provides investment education and access to a stock portal that allows employees to go to the stock market.
- CCP offers comprehensive benefit packages, competitive compensation, recruitment and retention bonuses, and generous paid time off.
- CCP offers company sponsored/paid STD/LTD/ Life/AD+D.
- You will have access to additional benefits including prepaid legal services, student loan assistance, Medicare workshops, and defensive driving.
- Part-time positions, job-sharing and flexible hours are often available for practitioners seeking these situations.
- These services are provided through CCP, so employees don’t have to coordinate these items independently.
