Breast Imaging Center of Excellence
The Breast Center at ImageCare in Latham, a division of Community Care Physicians, was the first outpatient non-hospital imaging center in the Capital Region to be designated a Breast Imaging Center of Excellence by the American College of Radiology. The Breast Center at ImageCare met very specific requirements established by the ACR and earned ACR accreditation in the following breast imaging modalities: mammography, stereotactic breast biopsy, breast ultrasound, and ultrasound guided breast biopsy. As an official Breast Imaging Center of Excellence, The Breast Center at ImageCare can offer patients a full spectrum of breast cancer screening and diagnosis services.

American College of Radiology Accreditation for Diagnostic Imaging
Community Care Physicians’ radiology department, ImageCare Medical Imaging, is accredited by the American College of Radiology (ACR) for breast imaging, mammography, CT scanning, lung cancer screening. An as accredited facility, ImageCare has met or exceeded nationally-accepted standards, has personnel that is well qualified to interpret your images, and the facility has the highest level of equipment, image quality, and patient safety.

Highly Complex Lab CLIA Certification
LabCare of Community Care Physicians is a full-service CLIA (Clinical Laboratory Improvement Amendment) Certified Laboratory available to our patients. CLIA established quality standards for laboratories to ensure the accuracy, reliability, and timeliness of patient test results.

NCQA — Physician Practice Connections® – Patient Centered Medical Home™
Community Care Physicians was the first medical group in the Capital Region to have primary care practices awarded recognition by the National Committee for Quality Assurance Physician Practice Connections® – Patient Centered Medical Home™ Program. Several of CCP’s primary care practices have earned this recognition. The Patient Centered Medical Home™ is a model of care that focuses on the relationship between the care team and the patient, where care is coordinated through patients’ primary care physicians. This designation affirms CCP’s commitment to the delivery of innovative, quality, compassionate healthcare and highlights our dedication to the wellbeing of our patients.

In 2021, Community Care Physicians was named one of Energage’s Top Workplaces USA 2021. This program recognizes companies from across the nation that prioritize a people-centered culture and do their part to make a difference. Being the first year for this program, it is an exceptional honor that CCP was recognized alongside other major companies from across all 50 states.

National Top Workplace
In 2013, Community Care Physicians was named a National Top Workplace based on feedback gathered through an objective employee survey. The survey was conducted by WorkplaceDynamics, LLP in conjunction with 30 leading regional newspapers throughout the country, including the Albany Times Union’s Top Workplaces in which Community Care Physicians participated. Community Care Physicians was ranked against the very best workplaces in major cities, including Chicago and Boston. CCP was ranked #55 nationwide in the largest survey of American workers of its kind.

Community Care Pediatrics – Saratoga was named the “Kid Picks Winner” for Favorite Pediatric Practice for 3 consecutive years in 2014, 2015 and 2016.

Community Care Pediatrics was named ‘Best Pediatricians in the Saratoga Region’ every year since 2009 by the readers of Saratoga Today newspaper in its annual “Best Of” awards. Saratoga Today selects the “Best of” winners by polling its readership each year using online voting.

Extraordinary Educators and Courage to Teach
Community Care Physicians Family Medicine residency physicians at Albany Family Medicine have been recognized for their dedication to teaching new doctors and their talent for creating innovative and effective residency programs and presented the Courage to Teach Award. The Parker J. Palmer Courage to Teach Award is presented by the Accreditation Council for Graduate Medical Education (ACGME).

HMSS National Davies Award for EHR
Community Care Physicians was one of four finalists across the country named for leveraging efficiencies of the electronic health record, or EHR. HIMSS, the Healthcare Information and Management Systems Society, is a company that collects and analyzes healthcare data related to IT processes and environments. Since 1994, this award has recognized and honored healthcare organizations for successful implementation of health information technology systems as well as a proven ability to derive value in the form of improved patient care.
