EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Ally Swiatowicz

Biography

Ally Swiatowicz is a Mental Health Counselor and works as a Behavioral Health Consultant (BHC) in Integrated Behavioral Health. BHCs at Community Care Physicians are embedded within our practices working side by side with your doctor and healthcare team in delivering you the best “whole person” healthcare possible. Ally is also one of the BHCs embedded on-site at Regeneron Pharmaceuticals, assisting with the behavioral health needs of Regeneron and its employees.

Ally has always had a passion for helping others. Through education and personal experiences, she has come to understand the connection between the mind, body, and spirit and the importance of healing each of these. Ally loves seeing patients learn the interconnectivity between these three aspects of their health and watching them grow with this knowledge. She believes collaboration, education, and motivation are most important when caring for patients.

Ally has experience with individual and group counseling, cognitive behavioral therapy, mindfulness-based therapy, as well as grief processing, stress management, and crisis intervention. She has worked with individuals with anxiety, depression, trauma, substance abuse, chronic illness, low self-esteem, and life transitions. She is particularly passionate about women’s issues. Ally enjoys being able to provide a compassionate space to help patients manage symptoms and promote self-healing. She is also a member of the New York Mental Health Counselors Association.

Outside of work, Ally enjoys being active through exercise and outdoor activities including rock climbing and kayaking. She loves cooking, gardening, and spending time with her husband and son.
