EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Biography

Robert Hawthorne is a Mental Health Counselor and works as a Behavioral Health Consultant (BHC) in Integrated Behavioral Health. BHCs at Community Care Physicians are embedded within our practices working side by side with your doctor and healthcare team in delivering you the best “whole person” healthcare possible. Rob is a member of the New York Mental Health Counselors Association.

Rob’s favorite part of working as a BHC is being able to engage with his patients.

In his free time, Rob volunteers at Code Blue Saratoga, a program of shelters that provides low-barrier emergency shelter to individuals who are homeless, and is a swim team official at the YMCA. He also enjoys fly fishing and spending time with his family.
