Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Hannah Barbieri

Biography

Hannah Barbieri is a Licensed Mental Health Counselor and works as a Behavioral Health Consultant (BHC) on our Integrated Behavioral Health team. BHCs at Community Care Physicians are licensed mental health providers and social workers. Many are embedded within our practices working side by side with your doctor and health care team in delivering you the best “whole person” healthcare possible. Hannah is a member of Ithaca College’s Psi Chi Society.

Hannah has experience with individual counseling with patients of all ages (children, adolescents, adults, and older adults), treating a multitude of behavioral health issues, including treating anxiety, depressed mood, stress management, sleep disturbances, and self-image/confidence. She has also worked with parents, families, and caregivers in several settings. Hannah has experience successfully integrating family members into the patient’s treatment plan, educating them on what the patient/their child needs and how to support/contribute to their well-being. At Integrated Behavioral Health, we also work with many patients suffering from relationship troubles, phase of life problems, and chronic pain.

Additional experience includes Family Treatment Court, Adult Protective Services, and a foster care program at Northern Rivers, where she worked closely with families.

Hannah always loved talking with others, offering guidance, and being very empathetic and a good listener. Hannah loves being a part of an amazing team of BHCs alongside our medical practitioners and particularly having the opportunity to work with children and adolescents. One thing Hannah really enjoys about her profession is working with her patients to normalize and validate their mental health struggles and see them feel better, improve and make progress!

When Hannah is not working, she enjoys spending time with her friends, family, and cat! Being a part of this field, she believes in self-care and taking downtime to recharge. One of her favorite hobbies and skills is makeup artistry which can be calming and therapeutic and a great creative outlet.
