EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Shannon Worden

Biography

Shannon Worden is a certified family nurse practitioner and a member of the American Association of Nurse Practitioners. She received her Master of Science in nursing from The State University of New York Polytechnic Institute in Utica, NY. Shannon began her studies at The State University of New York Buffalo in Buffalo, NY, earning her bachelor’s degree in nursing.

Shannon always had an interest in the sciences and loved helping people, so naturally, nursing was the way to go! She chose to specialize in family medicine because of the flexibility the field offers, allowing her to treat newborns to geriatric patients. Shannon has a particular interest in wound care, disease prevention, and wellness. She also enjoys the education aspect and building trusting relationships with patients and staff alike.

When Shannon isn’t working, she enjoys spending time with family, being outdoors, kayaking on Saratoga Lake, and reading.
