EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Rundi Constant

Biography

Rundi Constant is a board-certified family nurse practitioner and a member of the New York Nurse Practitioner Association. He received his Master of Science in nursing from Mount Saint Mary College in Newburgh, NY. Rundi began his studies at CUNY College of Staten Island in Staten Island, NY, earning his Bachelor of Science in nursing. He has a particular interest in preventative care and health promotion.

Rundi comes from a family of healthcare providers. Growing up, he always wanted to continue in his relatives’ footsteps. Becoming a registered nurse and finally, a nurse practitioner was always his passion. What Rundi loves most about his job is seeing his patients’ enthusiasm toward achieving the best version of themselves. He believes treating patients holistically is most important. Rundi strives to provide patients with the highest level of care possible, emphasizing education and promoting healthy behaviors. His care philosophy as a provider is always to remember that life is the most precious thing ever given to us. What brings him the most satisfaction professionally is seeing his patients achieve their health goals with his guidance and teachings.

Rundi is originally from New York City but moved to the Capital Region, specifically Glens Falls, for the family atmosphere. He enjoys hiking, biking, and traveling. Rundi also loves spending time with family and friends.
