Some of our patients have received letters in the mail from Change Healthcare regarding a data breach.
Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Biography

Originally from Plattsburgh, Rebecca Blake, PA-C, graduated from the State University of New York at Albany in 2009 with her bachelor’s degree in human biology and a double minor in psychology and public health. She then completed her Masters of Physician Assistant Studies at the State University of New York at Stony Brook in 2011. She worked for a short time in emergency medicine in the Saratoga Springs, NY region before discovering her passion for pediatrics at our Troy, NY office. Her areas of special interest include preventative care, adolescent medicine, and encouraging health literacy through the Reach Out and Read program.

In her personal life, Rebecca enjoys spending time with her husband, two sons, and two dogs. She also loves to be active outdoors and listen to live music.
