Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Carolyn Eaton

MD

Biography

Dr. Carolyn Eaton is a member of the American Academy of Family Physicians and the Alpha Omega Alpha Honor Medical Society. She received her medical degree from SUNY Upstate Medical University in Syracuse, New York. Dr. Eaton completed a family medicine residency at the University of Pittsburgh Medical Center, St. Margaret Memorial Hospital in Pittsburgh, Pennsylvania, and remained there to complete a fellowship in academic family medicine. Her areas of special interest include geriatrics. Dr. Eaton began her studies at Trinity University in San Antonio, Texas, earning her bachelor’s degree in mathematics.

Dr. Eaton returned to the Capital Region after training to join CapitalCare Family Practice Niskayuna, now known as Community Care Family Practice Colonie, in 2002. This is the same practice she worked at in her teenage years as a receptionist and file clerk!

Dr. Eaton is a native of Glenville, NY; she graduated from Burnt Hills-Ballston Lake High School. In her spare time, she enjoys spending time with her husband and two children, especially outdoors and in warm weather. She also enjoys cooking and gardening.
