Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Allison Graziadei

Biography

Each patient who comes before me is to be treated as a blank slate, and interventions commence from zero.

Dr. Allison Graziadei is a board-certified internist specializing in endocrinology, diabetes, and metabolism. She received her medical degree from Jacob School of Medicine and Biomedical Sciences, University at Buffalo in Buffalo, NY, and her residency at Jacob School of Medicine and Biomedical Sciences, University at Buffalo. Dr. Graziadei completed her fellowship at Warren Alpert Medical School of Brown University in Providence, RI, and is a member of the Endocrine Society, the American Association of Clinical Endocrinologists, and the American Diabetes Association. She earned her undergraduate degree at Union College in Schenectady, NY. Dr. Graziadei has specific medical interests that encompass all aspects of endocrinology and diabetes, emphasizing osteoporosis and thyroid care.

Dr. Graziadei decided to practice in the Capital Region because of the need for endocrinology services in Upstate New York. She chose to pursue medicine because both parents were in the healthcare field, and she grew up with an understanding of caring for those medically compromised. A pivotal moment that attracted her to endocrinology was witnessing grandparents who suffered from diabetic-related complications. Dr. Graziadei loves caring for patients who initially need health guidance and seeing and being part of their journey into wellness and a better lifestyle. She is professionally satisfied when a patient tells her how much better their life has become after following their treatment plan.

Dr. Graziadei is originally from Little Falls, NY. She loves going to the ocean every summer for quality time with her family and enjoys boating with them on local lakes. You can also find her during football season in the stands at Highmark Stadium – Go Bills!
