ImageCare consists of numerous outpatient imaging facilities, including two Breast Imaging Centers, as well as our hospital partners. Transition of Care
If you are admitted to the hospital, it is critical to have a clear plan in place for when you leave the hospital and to follow those discharge instructions. Our goal is to make your transition from the hospital to your home as smooth as possible. Transition of Care is available through your primary care office team at Community Care Physicians.

Our care management program is designed to help our patients who have chronic or complex health problems. Our care managers are here to ease the pressure and confusion, to improve your health, and to make navigating healthcare easier.
Patient Navigators
When seeking care in our area, you are not alone. We are pleased to have specially-trained navigators as part of Community Care Physicians to help patients and families through the complex world of cancer screening, diagnosis, treatment, and follow up care.

Telemedicine
In some instances, you can receive the care from us you want and need while remaining safely in your home using video visits with your doctor.
Patient Portal
Enrolling in our patient portal allows you to securely message your doctor’s office, request prescription refills online instead of calling the office, view test and lab results as soon as they become available, and set up proxy accounts for children and dependent adults.
Translation Services
You can find many multilingual medical professionals at Community Care Physicians. However, if your provider doesn’t speak your preferred language, we provide interpreter services across all CCP locations and for many languages through vendor partners. Just notify the front desk of your doctor’s office.
