Craig Maier, MD – Community Care Internal Medicine – Delmar
Dr. Maier is a board-certified physician who earned his medical degree from the State University of New York Upstate Medical University. He completed his residency through the Albany Medical Center’s Family Medicine Residency Program. Practicing at Delmar Internal Medicine, 250 Delaware Avenue, Delmar, Dr. Maier focuses on delivering sound, evidence-based medical care to his patients.

Lisa Thorn, MD – CapitalCare Family Practice Averill Park
A trusted provider in Rensselaer County for over 30 years, Dr. Thorn is a board-certified family physician who received her medical degree from Albany Medical College and completed her residency at St. Clare’s Hospital. She also serves as Medical Director at Eddy Village Green at Beverwyck. At CapitalCare Family Practice Averill Park, 3305 Route 43, Averill Park, Dr. Thorn provides comprehensive preventive care, sick visits, and wellness exams.

Lev Barats, MD – The Office of Dr. Lev Barats
Dr. Barats earned his medical degree from the University of Tennessee and completed a residency in internal medicine before serving as a primary care physician in the U.S. Air Force. His military experience included roles in NATO chemical warfare and nuclear monitoring, translation for European missions, and directing a regional medical center for the 20th USAF Medical Group. He now practices at The Office of Dr. Lev Barats, 14 Vista Boulevard, Slingerlands, where he focuses on preventive medicine and promoting healthy lifestyles.

Nicole Monserrate, MD – Community Care Internal Medicine – Delmar
Dr. Monserrate earned both her Bachelor of Science and Doctor of Medicine from Georgetown University and completed her internal medicine residency at Georgetown University Medical Center. She treats adult patients at Delmar Internal Medicine, 250 Delaware Avenue, Delmar, offering evidence-based care in a supportive, patient-focused environment.

Daniil Niyazov, MD – Community Care Internal Medicine – Delmar
Board certified in internal medicine, Dr. Niyazov received his medical degree from the Jacobs School of Medicine and Biomedical Sciences and completed his residency at Albany Medical Center. A lifelong advocate for community health, he began serving patients as a student volunteer at the Lighthouse Free Medical Clinic. Dr. Niyazov currently practices at Delmar Internal Medicine, 250 Delaware Avenue, Delmar.

Danielle Rooney, MD – Community Care Obstetrics and Gynecology
Dr. Rooney is board certified in obstetrics and gynecology. She received her medical degree and completed her residency at Albany Medical College after earning her bachelor’s degree in medical laboratory science from the University of Vermont. Before joining CCP, she practiced in a multidisciplinary OB/GYN and midwifery setting. She provides comprehensive women’s health services at Community Care Obstetrics and Gynecology, 6 Wellness Way, Suite 108, Latham.

Carol Greenblatt, DO – CapitalCare Pediatrics Troy
Dr. Greenblatt earned her medical degree from New York College of Osteopathic Medicine and completed her residency at Winthrop University Hospital. She specializes in adolescent medicine and adolescent brain development, helping families navigate the challenges of growing up. Dr. Greenblatt practices at CapitalCare Pediatrics Troy, 258 Hoosick Street, Suite 100, Troy.

Anthony Malone, MD – CapitalCare Pediatrics Troy & CapitalCare Developmental Pediatrics
Dr. Malone is double board certified in pediatrics and developmental-behavioral pediatrics, with deep expertise in caring for children with special needs. He received his medical degree from the University of Maryland School of Medicine and completed his pediatrics residency and developmental disabilities fellowship at Albany Medical Center.
