Welcome, Gabriella Izzo, MD, and Samantha Herman, CPNP-PC, PMHS, to Community Care Physicians

Community Care Physicians is pleased to welcome two new pediatric providers who are dedicated to delivering thoughtful, compassionate care to children and families across the Capital Region, fostering trust and confidence in our community.

Gabriella Izzo, MD – Community Care Pediatrics – Saratoga

Dr. Gabriella Izzo is a board-certified pediatrician who cares for patients from newborns through college age. She earned her medical degree from SUNY Upstate Medical University and completed her pediatric residency at the University of Connecticut/Connecticut Children’s Medical Center. She is a member of the American Academy of Pediatrics. Dr. Izzo has always been drawn to pediatrics, inspired by children’s resilience and the opportunity to support families through every stage of growth. She focuses on clear communication, individualized care, and building strong partnerships with parents. She has a special interest in working with new parents and young families, helping them feel confident from the newborn stage and beyond.

Originally from Rochester, Dr. Izzo is excited to call the Saratoga region home. Outside of work, she enjoys spending time with her family, being outdoors, and trying new restaurants. Read her full bio.

Samantha Herman, CPNP-PC, PMHS – CapitalCare Pediatrics – Troy

Samantha Herman is a certified pediatric nurse practitioner and Pediatric Primary Care Mental Health Specialist. She provides care from birth through young adulthood, with a focus on pediatric mental health, adolescent care, and complex conditions. She earned her Master of Science in Nursing from Seton Hall University and completed a pediatric mental health fellowship through The Ohio State University’s KySS program. She is a member of the National Association of Pediatric Nurse Practitioners. Samantha’s path into healthcare was shaped by personal caregiving experiences, inspiring her commitment to supporting families through complex health challenges. She is passionate about building strong relationships with patients and families, especially when answers have been difficult to find, and prioritizes listening to ensure thoughtful, individualized care.

Outside of work, Samantha enjoys spending time with her family. She and her family are foster parents to local youth and stay busy with music, dance, and their household of pets. Read her full bio.
