We are able to care for patients of all ages. Made up of Family Medicine providers and “Med-Peds”, or providers that are double-board certified in Internal Medicine and Pediatrics, you can stay with any of the providers within the Division of Family Care from birth until your golden years.
Our family care offices utilize modern technology to enhance efficiency and allow for greater patient-centered care. All of our offices are connected to you and to each other by means of Community Care’s Electronic Health Record, putting your critical health information at the fingertips of any Community Care practitioner who treats you. And through our patient portal, mycareDOT™ powered by Follow My Health™, you too have access to your doctor and to your medical record via computer, tablet or smart phone.
As a multispecialty medical group, your family physicians have the support of over 30 different medical specialties for specialized care you may require. Many of these specialists are arranged in convenient, multispecialty health parks around the Capital Region, so your care is both coordinated and convenient.
