Community Care Family Practice Colonie is a team of board-certified family physicians, advanced practitioners, and skilled, compassionate staff focused on providing the highest quality of care for you and your family. Our practice has been Recognized by the National Committee for Quality Assurance (NCQA) as a New York State Patient-Centered Medical Home (NYS PCMH). Our office is located in a plaza at 501 New Karner Road in Albany, NY. The location is very accessible for our patients with ample, well-lit parking. There are no stairs for patients to climb, no ramps required, and the building is handicap accessible. We offer telemedicine, on-site lab services Monday through Friday from 8:00 am to 4:30 pm, and online access to our practice through our patient portal.

1. Office Visit – Need to address something new or check in on an existing concern for which you were already seen at our office? Choose the Office Visit appointment to schedule.
2. Pre-Op – Need to get clearance for an upcoming procedure or operation? Choose the pre-op appointment to schedule.
