Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.

Community Care Family Medicine cares for all ages, including newborns, pediatrics, and adults. Located in a multi-specialty medical facility on Route 9 in Clifton Park, NY, Community Care Family Medicine offers numerous advantages over a traditional primary care practice. In addition to our extended hours of operation, we have surrounded ourselves with many subspecialties and services that would otherwise be unavailable at a standalone practice. This results in better care for you and your family in terms of convenience, service, hours, and on-site subspecialty consultation.

Our board-certified family physicians, nurse practitioner, and staff are dedicated to providing our patients with the highest quality, evidence-based health care services. We are trained to diagnose and treat a wide range of acute and chronic medical conditions for people of all ages. Dr. Eric Schnakenberg is a certified provider of medical marijuana as well. Our staff works with you as a team to provide each of our patients with “whole person” health care. Comprehensive care is our goal of providing complete wellness. By getting to know each patient beyond their immediate clinical needs, we can offer our patients customized health management services.

A provider is always on call and available for emergencies after hours, and patients can visit our CCP Urgent Care Center in Latham when directed by our team.

Community Care Family Medicine has been Recognized by the National Committee for Quality Assurance (NCQA) as a New York State Patient-Centered Medical Home (NYS PCMH). This designation affirms our commitment to the delivery of innovative, quality, compassionate healthcare and highlights our dedication to the wellbeing of our patients.
