Gloria Diaz is a Mental Health Counselor and Behavioral Health Consultant (BHC) on our Integrated Behavioral Health team at Community Care Physicians. With a background in public health research and experience supporting individuals with anxiety, depression, PTSD, and chronic health conditions, Gloria is passionate about providing “whole person” care alongside our medical teams. Her work is informed by deep empathy and a desire to help patients process their experiences and build fulfilling lives. Gloria finds joy in witnessing her patients grow in confidence, seeing herself as a supportive guide on their healing journey. Introducing Brittney Lillie, FNP-C
Brittney Lillie, FNP-C, is a family nurse practitioner specializing in family medicine and pediatrics, with a passion for holistic, patient-centered care. Originally from Conquest, NY, she was inspired by early experiences with her pediatrician and now focuses on preventive medicine, chronic disease management, women’s health, and asthma care. Brittney holds degrees from the University of Buffalo, St. Joseph’s College of Nursing, and Le Moyne College, where she also earned her Master’s as a Family Nurse Practitioner. Certified through the ANCC, she values collaboration and long-term relationships with her patients. Outside of work, she enjoys crocheting, Dungeons & Dragons, reading, and swimming. About Integrated Behavioral Health
At Integrated Behavioral Health (IBH), our team offers patients services for behavioral changes and overall behavioral health including mental health, substance abuse, and lifestyle medicine. Our behavioral health consultants are integrated into the majority of our primary care practices at CCP, and our services are also available to the community without a referral. Integrated Behavioral Health is available for patients of all ages who are in need of behavioral health or psychiatry services.
