EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Biography

Dr. Tami Seaman is board certified in family medicine. She loves caring for patients through the entire life spectrum and the connections she is able to make with patients and their families. She received her Doctor of Medicine from the University at Buffalo School of Medicine and Biomedical Sciences in Buffalo, NY, and completed a family medicine residency with SUNY Buffalo at Millard Filmore Suburban Hospital. Dr. Seaman began her studies at Cornell University in Ithaca, NY, earning her Bachelor of Arts.

Originally from Johnstown, NY, Dr. Seaman’s family moved to Clifton Park, and she graduated from Shenendehowa High School. Dr. Seaman likes nothing more than being outside, hiking, biking, or just in her yard tending the flowers. She also cherishes time spent with her almost grown-up children and her husband.
