Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Mary Phelps

Biography

Mary Phelps is a board-certified family nurse practitioner and a member of the American Nurses Association. She received her Master of Science in nursing from SUNY Polytechnic Institute in Utica, NY. Mary began her studies at SUNY Oneonta in Oneonta, NY, earning a Bachelor of Science in biology. She worked in the field of cardiology for ten years. She is interested in heart health, women’s health, movement disorders, and disease prevention.

Mary worked as a Patient Care Technician at a local hospital throughout college. She saw the positive differences nurses could make in a patient’s care which solidified her decision to attend nursing school. Mary received her associate’s degree in nursing from Maria College in Albany, NY, and completed her bachelor’s degree online through SUNY Delhi in Delhi, NY. She always wanted to be more involved in the patient’s care plan, which made her pursue an advanced nursing degree.

Mary had always known that helping people was something she had wanted to do since she was young. While working in a hospital to gain experience during college, she learned about the true impact healthcare professionals have on their patients. Nursing allows Mary to provide a strong foundation of caring for others and can help patients further with disease management. She loves the ability to communicate and establish relationships with patients. Mary always found pride in connecting with patients and learning about their families, career, and likes and dislikes.

Mary believes it is vital for quality care to let patients know they are important. She values the effects of open communication as well as patient advocacy. She wants her patients to feel heard and know they control their health. As a hardworking family nurse practitioner, Mary will ensure her patients receive the best care possible, listen to their concerns, and address them. Over the years, she has cared for patients from all walks of life. Her goal is to treat each patient with respect and give them the attention and care they deserve.

Mary is originally from Albany, NY. She is very family oriented and spends a lot of her time outside work spending time her family and friends. Music has played a significant role in Mary and her family’s life. Her father is in a bluegrass band, and Mary plays the guitar, sings, and loves Broadway musicals. She also enjoys going on walks with her husband, son, and their dog Piper.
