Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Kevin Pachucki

Biography

Kevin Pachucki is a certified physician assistant. His medical interests blossomed at an early age and further developed in a first-of-its-kind medical technology course offered at his high school. This led him to pursue a medical provider track that brought him to Boston, MA (and while he would vehemently deny it, the fact that Northeastern University was less than a mile from Fenway Park and his Red Sox was likely the biggest pull towards Boston for college). Luckily, his experience only furthered his enthusiasm for medicine and science. He left eight years later with a bachelor’s degree in medical laboratory science, several years of research at Boston Children’s Hospital, and a master’s degree in Physician Assistant Studies.

In the years that have passed, Kevin has been practicing medicine at CapitalCare Family Practice Slingerlands of CCP, with a passion for addiction therapy, pediatric care, and preventative medicine. He also provided adolescent medical care for years at the Parson’s Child and Health Center. Outside the office, Kevin can be found spending time with his wife and daughter, in his woodshop, or nurturing his avid running passion.
