EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Biography

Dr. Joanne Bifaro is board certified in family medicine, and she loves practicing in this field! She earned her medical degree from Albany Medical College in Albany, NY, as well as a Master of Science from Union Graduate College and a Bachelor of Science from Union College in Schenectady, NY. Dr. Bifaro completed her residency at the University of Massachusetts Worcester.

In her patient care responsibilities, Dr. Bifaro sees patients at Albany Family Medicine in Albany, NY, where she enjoys providing care to patients of all ages, including providing newborn care, gender-affirming treatment, prenatal care, and medication-assisted treatment for substance use disorders. Additionally, she has a passion for medical education, teaching the next generation of family physicians in her role as associate professor of Family Medicine at Albany Medical College. When she’s not working, you may find Dr. Bifaro exploring one of the Capital Region, NY’s many playgrounds or outdoor activities with her family.
