EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Biography

Dr. Jill Gradner is board-certified in internal medicine and obesity medicine at Community Care Internal Medicine Clifton Park. She received her medical degree from New York Medical College in Valhalla, NY. Dr. Gradner also holds a bachelor’s degree from the State University of New York at Stony Brook in Stony Brook, NY, and the State University of New York College of Medicine in Brooklyn, NY. She completed an OB/GYN residency at Westchester County Medical Center in Valhalla, NY, and an internal medicine residency at Lenox Hill Hospital in New York, NY.
