Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Gina Ptak

Biography

Your present circumstances do not determine where you can go; they merely determine where you start.

Gina Ptak is a certified family nurse practitioner and a member of the American Association of Nurse Practitioners. She received her Master of Science in nursing, family nurse practitioner from The Sage Colleges in Albany, NY. Gina began her nursing studies at Albany Memorial School of Nursing in Albany, NY, earning her Associate of Science. She also received a Bachelor of Arts in communication from SUNY Albany in Albany, NY, and an Associate of Science in business administration from Schenectady County Community College in Schenectady, NY. Gina treats patients 16 years of age and older at Community Care Internal Medicine Clifton Park.

Gina has gained a wealth of experience working in the ER and six years as a traveling nurse. What brings her the most satisfaction professionally is helping patients achieve their goals, whether weight loss, improving diabetic or blood pressure control, or even taking a natural approach to treating their ailments.

Gina decided to pursue medicine in her early 20s. She has a passion for positively impacting lives through healthcare, a demanding yet rewarding career, and she loves doing so. During her time working in the ER, Gina witnessed the declining health of our community and knew she wanted to be on the frontline helping patients. She cares for each patient holistically. Gina believes in being empathic, listening and communicating effectively, and understanding that her patients have a right to shared decision-making regarding their personal health and treatment plans. She has a particular interest in weight loss and preventative medicine. Throughout her training, Gina has learned that everyone can make positive changes in their lives, whether emotionally, mentally, physically, or spiritually with the proper support.

Outside of medicine, Gina enjoys spending time with her husband and daughter. She is also a real estate photographer.
