Community Care Doctors Recognized as Hudson Valley “Top Docs” for 2024

Dr. Lev Barats, Internal Medicine

After completing medical school at the University of Tennessee, and finishing his residency in Internal Medicine, Dr. Barats joined the US Air Force as a primary care physician. He provided care to active duty and the dependent population. During his service, he was also involved with the NATO chemical warfare and nuclear monitoring program and served as a translator for several European missions. His medical duties included directorship at a regional medical center of the 20th USAF Medical Group. Dr. Barats currently practices at The Office of Dr. Lev Barats, 14 Vista Boulevard, Slingerlands, where he is passionate about preventive care and promoting a healthy lifestyle for his patients.

Dr. Lisa Bevilacqua, Family Medicine

Dr. Bevilacqua is double board certified in family medicine and aesthetic medicine. She received her Doctor of Osteopathic Medicine at the New York Institute of Technology College of Osteopathic Medicine. Following this, Dr. Bevilacqua completed an osteopathic rotating internship at St. Clare’s Hospital. She continued her training at Albany Medical Center, completing a family medicine residency. Dr. Bevilacqua has a particular interest in prevention and addiction medicine. She sees patients at CapitalCare Family Practice Slingerlands, 1882 New Scotland Road, Suite 200, Slingerlands, providing family medicine and aesthetic medicine.

Dr. Carol Greenblatt, Pediatrics

Dr. Greenblatt completed her medical degree at New York College of Osteopathic Medicine and her residency at Winthrop University Hospital. She is interested in adolescent medicine and the growth and development of the adolescent brain. Dr. Greenblatt strives to help patients and parents navigate such a difficult time in their lives and hopes to comfort them. She is one of the senior physicians at CapitalCare Pediatrics Troy, 258 Hoosick Street, Suite 100, Troy.

Dr. Craig Maier, Internal Medicine

Dr. Maier is a board-certified physician who earned his medical degree from the State University of New York Upstate Medical University. He completed his residency through the Albany Medical Center’s Family Medicine Residency program. Dr. Maier sees patients at Delmar Internal Medicine, 250 Delaware Avenue, Delmar, where he is interested in practicing sound science and evidence-based medicine.

Dr. Anthony Malone, Pediatrics

Dr. Malone is board certified in pediatrics and developmental-behavioral pediatrics. He has extensive experience in the care of special needs children. He received his Doctor of Medicine from the University of Maryland School of Medicine and began his studies at Trinity College. Dr. Malone completed his residency in pediatrics and his fellowship in developmental disabilities at Albany Medical Center.  He treats patients at CapitalCare Developmental Pediatrics, 6 Wellness Way, Suite 101, Latham, and CapitalCare Pediatrics Troy, 258 Hoosick Street, Suite 100, Troy.

Dr. Nicole Monserrate, Internal Medicine

Dr. Monserrate earned her Bachelor of Science and Doctor of Medicine from Georgetown University. She completed her internal medicine residency at Georgetown University Medical Center. She treats adult patients at Delmar Internal Medicine, 250 Delaware Avenue, Delmar, where she provides evidence-based care in a comfortable environment.

Dr. Daniil Niyazov, Internal Medicine

Dr. Niyazov is a board-certified physician in internal medicine. He earned his medical degree from the Jacobs School of Medicine and Biomedical Sciences. His residency program was in internal medicine at Albany Medical Center. Dr. Niyazov volunteered as a student at the Lighthouse Free Medical Clinic, giving back as much as possible to the community around him. He currently practices at Delmar Internal Medicine, 250 Delaware Avenue, Delmar.

Dr. Danielle Rooney, Obstetrics and Gynecology

Dr. Rooney is board certified in obstetrics and gynecology by The American Board of Obstetrics and Gynecology. She received her medical degree from Albany Medical College and completed her residency in the OB/GYN department at Albany Medical College. Dr. Rooney began her studies at the University of Vermont, earning a bachelor’s degree in medical laboratory science. Prior to joining Community Care, Dr. Rooney worked in a private practice that offered obstetrics, gynecology, and midwifery care. She sees patients at Community Care Obstetrics and Gynecology, 6 Wellness Way, Suite 108, Latham, providing full spectrum of OB/GYN care.

Dr. Lisa Thorn, Family Medicine

Dr. Thorn has served patients in Rensselaer County for over 28 years as a board-certified family physician. She received her medical degree at Albany Medical College and completed a family medicine residency at St. Clare’s Hospital. Dr. Thorn is also the Medical Director at Eddy Village Green at Beverwyck. She practices at CapitalCare Family Practice Averill Park, 3305 Route 43, Averill Park, offering routine preventive care, sick visits, wellness exams, and more.

Dr. Rommel Tolentino, Internal Medicine + Pediatrics

Dr. Tolentino is board certified in pediatrics and internal medicine and fluent in Tagalog. He cares for patients from infancy through advanced age. Dr. Tolentino received his medical degree from the University at Buffalo Jacobs School of Medicine and Biomedical Sciences. He completed his internship and residency training in internal medicine and pediatrics at Albany Medical Center. Dr. Tolentino attended SUNY Buffalo, earning his Bachelor of Science in biochemistry. He is a Fellow of the American Academy of Pediatrics and the American College of Physicians. Dr. Tolentino treats patients of all ages at Schodack Internal Medicine and Pediatrics, 35 Empire State Boulevard, Castleton-On-Hudson.
