We are excited to announce the addition of three exceptional providers to the Community Care family: Vanessa Gagne, FNP-BC, to Community Care Internal Medicine – Clifton Park, Dr. Mai Stewart to our radiology team, and Savanah Leber, PA-C, to Community Care General Surgery.
Say Bonjour to Vanessa Gagne, FNP-BC
Vanessa isn’t just another nurse practitioner—she’s a listening, healing ninja. A board-certified FNP with over a decade’s worth of registered nursing grit and heart, Vanessa specializes in internal medicine for adult patients. She holds both her MSN and BSN from SUNY Polytechnic Institute, Utica. Originally from Montreal and now proudly rooted in upstate New York (Greenwich has been her home for 12 years!), her journey into caring began when her son spent two months in the NICU—talk about first-hand education in compassion. Vanessa’s philosophy? “Mind, body, and soul—everything is connected.” And yes, she means it. Outside the office, she hits the hiking trails and kayaks. Read her bio to learn more.
