EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Savanah Leber

Biography

Savanah Leber is a certified physician assistant specializing in general surgery and a proud member of the New York State Society of Physician Assistants. She earned her Bachelor of Arts in biology from the State University of New York at Albany, followed by a Master of Science in Physician Assistant Studies from Albany Medical College.

At Community Care General Surgery in Latham, NY, Savanah provides surgical care for patients ages 16 and older. Her clinical interests include diabetes, functional medicine, and robotic surgery. She also sees patients at St. Peter’s Hospital in Albany, NY.

Savanah’s passion for medicine began early. While completing her undergraduate studies, she worked as a nurse’s aide on a physical rehabilitation unit. This experience reinforced her desire to help others and deepened her love of science and patient care. Drawn to surgery throughout her training, she finds particular satisfaction in assisting with open abdominal surgeries and performing day-one post-operative checkups—moments that combine technical skill with direct patient connection. She believes that respect, attentive listening, and clear communication are essential for delivering high-quality healthcare and strives to treat every patient as she would want to be treated herself.

A native of Colonie, NY, Savanah is active in Breakthrough T1D, a global organization dedicated to research and advocacy for Type 1 Diabetes. Outside of medicine, she enjoys cooking, gardening, hiking, and traveling.
