Community Care Physicians is proud to welcome three talented providers to our growing team: Marie (Therese) Paggao, FNP-BC, Madison Donnelly, PA-C, and Joana Sun, FNP-C. Each brings unique expertise, a patient-centered approach, and a commitment to helping our patients thrive.
Marie (Therese) Paggao, FNP-BC — Delmar Internal Medicine

Therese Paggao is a board-certified family nurse practitioner fluent in Tagalog, specializing in adult care with a focus on chronic disease management and prevention. She earned her Master of Science in nursing from United States University, her Bachelor of Science in nursing from Chamberlain University, and her associate degree in nursing from Union County College. Therese is also a certified nephrology nurse and clinical transplant coordinator, and she is an active member of the American Nurses Association and the American Association of Nurse Practitioners.
Therese knew from a young age that she wanted to pursue a career in medicine. She finds deep fulfillment in educating patients and empowering them to take better care of themselves, particularly those managing chronic conditions. Her approach addresses the whole person—physical, mental, and emotional well-being—and she is known for being patient, empathetic, and an attentive listener. Born and raised in the Philippines, Therese immigrated to the U.S. at age 17 and lived in New Jersey for more than 20 years before moving to the Capital Region. She has participated in two overseas medical missions and enjoys cooking and baking, as well as crocheting and knitting, interior decorating, and crafting. Read her full bio.
Madison Donnelly, PA-C — Community Care OB/GYN

Madison Donnelly is a certified physician assistant specializing in obstetrics and gynecology. Originally from East Brunswick, NJ, Madison earned both her undergraduate degree from Hofstra University and her Master of Physician Assistant Studies from Hofstra Northwell School of Nursing and Physician Assistant Studies. Madison’s passion for medicine began in high school and has grown through a career spanning pediatrics to geriatrics. She brings this broad experience to women’s health care, with clinical interests including patient advocacy, supporting NICU families, and addressing the unique needs of historically underrepresented groups. She is an active member of the American Academy of Physician Associates, the New York State Society of Physician Associates, and the Association of PAs in Obstetrics and Gynecology.
Madison approaches each patient interaction with empathy and thoughtfulness, listening carefully and addressing concerns with clarity. She strives to provide care with dignity, compassion, and integrity, supporting patients through every stage of their health journey. Outside of work, Madison enjoys baking, crafting, and donating hand-knitted baby blankets to local charities. Read her full bio.
Joana Sun, FNP-C — Community Care Family Practice Colonie

Joana Sun is a certified family nurse practitioner who provides care to patients of all ages. She earned her Master of Science in nursing from UMass Medical School and her Bachelor of Science in biomedical science and psychology from Marist College. Joana is a member of the American Association of Nurse Practitioners and speaks conversational Mandarin Chinese. Her early experiences as a nurse inspired her to become a nurse practitioner so she could advocate for patients and ensure their symptoms and concerns are heard. Joana emphasizes listening and whole-person care, creating a safe space where patients feel supported and informed. Her clinical interests include women’s health, particularly endometriosis and pelvic pain, and integrating traditional and holistic approaches to care.
Originally from Connecticut, Joana spent a decade in the Bay Area before returning to the Capital Region. She enjoys tennis, weightlifting, exploring national parks, cooking, and photography, and she looks forward to participating in community programs like ACEing Autism. Read her full bio.
