Community Care Physicians is pleased to welcome Kaden Sady, FNP-BC, and Ane Kline, PA-C, to our primary care teams in Albany County. Both providers bring strong clinical backgrounds, a collaborative approach, and a deep commitment to patient-centered care within their practices.
Kaden Sady, FNP-BC — Delmar Internal Medicine
Kaden Sady is a board-certified family nurse practitioner at Delmar Internal Medicine. She earned both her Master of Science and Bachelor of Science in Nursing from SUNY Polytechnic Institute, after beginning her nursing education at Mohawk Valley Community College. Kaden’s passion for healthcare started as a teenager after witnessing the compassionate care her grandmother received during her battle with pancreatic cancer. Before becoming a family nurse practitioner, she worked as a registered nurse with experience in psychiatry and case management. During the COVID-19 pandemic, she felt called to further her education to practice with greater autonomy and provide more comprehensive care. Kaden’s favorite part of her work is building meaningful relationships with patients and partnering with them to understand their health and achieve their goals. She emphasizes holistic, compassionate, and patient-centered care, creating a safe and supportive environment where patients feel comfortable sharing their concerns.
Originally from Central New York, Kaden moved to the Capital Region in 2018 and is proud to care for the Delmar community. Outside of work, she enjoys spending time with her family, parenting, baking, and getting outdoors. Read her full bio.
Ane Kline, PA-C — Albany Family Medicine
Ane Kline is a certified physician assistant specializing in women’s health at Albany Family Medicine, where she provides care for patients of all ages. She earned her Master of Science in Physician Assistant Studies from SUNY Upstate Medical University and her Bachelor of Arts in Psychology from Ohio Wesleyan University. Ane’s interest in healthcare began in high school and was shaped by both personal health experiences and a growing appreciation for patient advocacy and collaboration. She values building strong relationships with patients and believes that listening carefully and taking concerns seriously are essential to providing high-quality care. Her clinical experience includes gender-affirming hormone therapy and long-acting reversible contraception (LARC) insertion and removal. She is committed to transparency and thoughtful care, and she is comfortable asking questions and seeking additional insight when needed to ensure the best outcomes for her patients.
Originally from the Finger Lakes region, Ane enjoys building LEGO sets, spending time with her puppy, baking cookies, learning about astrology, and collecting Pokémon cards. Read her full bio.
