Welcome to InfusionCare of Community Care Physicians, dedicated to delivering exceptional infusion services for our community.

You can find our infusion suite at the cutting edge medical facility Wellness Way off Route 9 in Latham, NY. We provide our patients high-quality, non-oncology, multispecialty infusion support in areas such as rheumatology, gastroenterology, neurology, and soon, iron and intravenous immunoglobulin (IVIG). Under the medical direction of Dr. Jessica Chapman, InfusionCare’s clinicians administer the medications prescribed by the patient’s physicians and oversee the infusion process, but we do not manage the patient’s medical conditions directly.

Wellness Way is conveniently located just off the Northway and Rt. 9 in Latham. CDTA bus route #182 will take you to the Wellness Way – CDPHP/Community Care Physicians stop. 6 Wellness Way was previously called 6 Autopark Drive. If ride shares such as Uber, Lyft, or Medi Cab services don’t yet recognize the new address, please use 6 Autopark Drive, Latham, NY 12110 to get to Wellness Way.
