Dr. Jessica Chapman left Community Care Physicians February 2, 2026. Dr. Michaels, Marissa Page, NP, and Barbara Nguyen, PA-C, will continue practicing at Community Care Rheumatology until April 1, 2026, at which point they will all transition to Albany Med Health System, and Community Care Rheumatology will close. Dr. Michaels, Marissa Page, and Barbara Nguyen will continue to offer the same rheumatology care in the same Latham location under a new practice name as Albany Med providers: Albany Med Rheumatology & Infusion Center, 6 Wellness Way, Suite 113, Latham, NY 12110.
Albany Med already has a well-established rheumatology division with expanded specialty resources and a rheumatology fellowship program through which many rheumatologists have trained over the years. This transition ensures continued access to comprehensive rheumatology care in our community, support for the existing rheumatology team, and long-term stability for continued patient care.

Community Care Rheumatology welcomes patients aged 16 years and older. We diagnose and treat all types of arthritis and joint conditions, as well as a variety of other musculoskeletal, autoimmune, and connective tissue disorders.
Our experienced rheumatology providers, Dr. Victoria Michaels, Marissa Page, NP, and Barbara Nguyen, PA-C, are passionate about helping each patient who comes through our doors, providing individual diagnosis and treatment.
Community Care Rheumatology offers on-site imaging, lab, and pharmacy. Our team also offers comprehensive bone health support through our Bone Health Program to keep your bones strong and prevent serious injuries.
While we don’t currently offer walk-in hours, our practitioners are easily accessible to care for patients in multiple ways, including over the phone, via telemedicine, or in the office located just off of Route 9 in Latham, NY.
Please note, we understand that at times you may need to change your appointment. If it is necessary to cancel an appointment, patients are required to call or leave a message at least 24 hours before their appointment time. Failure to do so may result in a “same-day cancellation” fee.

Wellness Way is conveniently located just off the Northway and Rt. 9 in Latham. CDTA bus route #182 will take you to the Wellness Way – CDPHP/Community Care Physicians stop. 6 Wellness Way was previously called 6 Autopark Drive. If ride shares such as Uber, Lyft, or Medi Cab services don’t yet recognize the new address, please use 6 Autopark Drive, Latham, NY 12110 to get to Wellness Way.
