Community Care Rheumatology welcomes patients 16 years of age and older. We diagnose and treat all types of arthritis and joint conditions as well as a variety of other musculoskeletal, autoimmune and connective tissue disorders.

Our experienced rheumatologists, Dr. Jessica Chapman and Dr. Victoria Michaels, along with Barbara Nguyen, PA-C, Marissa Page, NP, and the rest of the Community Care Rheumatology team, are passionate about helping each patient who comes in our doors, providing individual diagnosis and treatment.

Community Care Rheumatology offers on-site imaging, lab, pharmacy, and access to in-office infusions and a Fast Track Osteoporosis Infusion Program through InfusionCare of Community Care Physicians, under the medical directorship of Dr. Jessica Chapman.

While we don’t currently offer walk-in hours, our practitioners are easily accessible to care for patients in multiple ways, including over the phone, via telemedicine, or in the office located just off of Route 9 in Latham, NY.

Please note, we understand at times you may need to change your appointment. If it is necessary to cancel an appointment, patients are required to call or leave a message at least 24 hours before their appointment time. Failure to do so may result in a “same-day cancellation” fee.
