Update on 5/31/23 at 11:00am
This issue has been resolved. Please read below on how to reconnect with us if you opted-out of text communications during this time.
Yesterday late afternoon, you may have received email, text or voice messages from our offices that were sent in error (for example, blank forms to complete that weren’t relevant to you or your visit). While we acknowledge that this may have caused a minor inconvenience, we want to emphasize that no Protected Health Information (PHI) was included in any of these messages.
We are pleased to inform you that our dedicated technical team promptly identified and resolved the issue. We sincerely apologize for any confusion or inconvenience this incident may have caused you. We understand the frustration of receiving unnecessary messages, and we are committed to preventing such incidents from happening again in the future.
Furthermore, we would like to inform you that during the resolution process, our appointment reminder service was temporarily halted. This issue has been completely resolved, and you will resume receiving your appointment reminders just as before.
If you were among those affected by this incident, we genuinely appreciate your understanding and patience throughout this matter. Your support and cooperation mean a great deal to us. If you chose to opt out of our communications during this occurrence and now wish to resume receiving messages relevant to your care (including appointment reminders), please text the word START to 1-385-336-6206 to opt back in.
Thank you for your continued trust in Community Care Physicians.
