Biography

Vanessa Gagne is a board-certified family nurse practitioner who provides compassionate, individualized care to adult patients at Community Care Internal Medicine Clifton Park. With a strong foundation in health promotion and disease prevention, she is deeply committed to building collaborative, respectful relationships with every patient she sees.

Vanessa earned both her Master’s and Bachelor of Science in nursing from SUNY Polytechnic Institute in Utica, NY. She is a member of the Nurse Practitioner Association (NPA) and speaks conversational French.

Originally from Montreal, Quebec, Vanessa has spent most of her life in upstate New York and has called Greenwich home for the past 12 years. A deeply personal experience inspired her path to nursing—after her son spent two months in the NICU, she witnessed firsthand the power of compassionate, skilled care. That experience fueled her desire to support others during their most vulnerable times.

With more than a decade of experience as a registered nurse, Vanessa brings empathy, clinical expertise, and a whole-person approach to every encounter. She believes the most important part of care is listening—not just hearing symptoms, but truly understanding her patients’ concerns, goals, and life context. “Mind, body, and soul—everything is connected,” she says.

Whether helping patients manage chronic conditions or focusing on preventive care, Vanessa finds meaning in the small moments: connecting with patients, building trust, and empowering them in their health journey.

Outside of work, she enjoys spending time outdoors, including hiking and kayaking.
