Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Stephanie Jorgensen

MD, FAAP

Biography

Dr. Stephanie Jorgensen is board certified by the American Board of Pediatrics. She received her medical degree from Albany Medical College in Albany, New York, and her bachelor’s degree from Siena College in Loudonville, New York. She completed her residency as Associate Chief-Resident at Albany Medical Center Hospital from 1997 to 2000. Dr. Jorgensen belongs to the American Academy of Pediatrics, MVMA Physicians, and is an Instructor for the Neonatal Resuscitation Program.

Dr. Jorgensen worked at Glens Falls Pediatrics from 2000 to 2010. During this time, she was also the District Physician for Lake George Central Schools. She began working at Community Care Physicians in September 2010.
