EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Sierra Moon

Biography

Dr. Sierra Moon is a resident physician at Albany Family Medicine in Albany, NY. Her journey began in Southern California, where she pursued her undergraduate studies at UCLA in Los Angeles, CA, focusing on human biology and society. During this time, she had the privilege of working with children with Autism Spectrum Disorders and in women’s health. Dr. Moon’s path then led her to Washington state for medical school, where she met her partner, who is from Syracuse, NY. Her passion for family medicine is far from a cliche. It’s a lifelong aspiration to develop deep connections with her patients and support them in all aspects of their lives. Dr. Moon is particularly drawn to osteopathic medicine, women’s health, mental health, and serving underserved populations. The comprehensive nature of family medicine allows her to explore these interests and be there for her patients at every stage of life. Outside of medicine, Dr. Moon loves spending her free time outdoors with her dog, Olive, whether hiking, camping, swimming, or trying new restaurants. She also loves to read, crochet, cook, and play board games.
