Some of our patients have received letters in the mail from Change Healthcare regarding a data breach.
Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Shannon O’Meara

Biography

Shannon O’Meara is a board-certified family nurse practitioner. She received her Master of Science in nursing, majoring in family nurse practitioner, at the State University of New York Institute of Technology in Utica, New York. Shannon began her nursing studies at the University of Miami School of Nursing and Health Studies in Coral Gables, Florida, earning her Bachelor of Science in nursing. She also completed a bachelor’s degree in biology from the State University of New York at Albany in Albany, New York. Shannon completed her training in family medicine, internal medicine, emergency medicine, women’s health, and pediatric medicine.

Born and raised in Troy, New York, Shannon is interested in family practice, primary prevention, and wellness care. When she’s not caring for patients, Shannon enjoys exercising, particularly running,  and spending time with her family.
