EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Biography

Rachel Whitney is a board-certified family nurse practitioner certified by the American Nurses Credentialing Center. She earned her Master of Science in nursing from Sacred Heart University in Fairfield, CT, and her Bachelor of Science in nursing from SUNY Plattsburgh in Plattsburgh, NY, after beginning her education at SUNY Adirondack in Queensbury, NY.

Prior to becoming an FNP, Rachel worked for over 13 years as a medical-surgical nurse in both rural and Level I trauma hospital settings. This experience shapes her patient-centered approach to care, emphasizing careful listening, respect, and compassionate treatment.

Originally from the Burnt Hills–Ballston Lake area, Rachel is proud to serve the community she calls home. Outside of work, she enjoys spending time with her family, traveling, and supporting her son’s ice hockey.
