Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Biography

A dedicated physician and a willing patient are a powerful team to achieve one’s greatest health.

Dr. Nicholas J. Fusella is board certified by the American Board of Family Medicine and the American Osteopathic Board of Family Physicians. He is a member of the American Medical Association, the American Board of Family Medicine, the American Academy of Family Physicians, the American Osteopathic Board of Family Physicians, and the American College of Osteopathic Family Physicians. Dr. Fusella received his medical degree from NYIT College of Osteopathic Medicine in Old Westbury, NY, and completed his internship and residency at Ellis Hospital in Schenectady, NY. He began his studies at Siena College in Loudonville, NY, earning his Bachelor of Science in biology. Dr. Fusella has a particular interest in Type 2 Diabetes, Hypertension, and Hyperlipidemia.

Dr. Fusella’s upbringing is what fostered his desire to pursue medicine. His father is a local family physician who has dedicated his professional life to his patients and their wellbeing. With his father as a role model, he knew he too wanted to pursue medicine to help patients achieve the healthiest versions of themselves. Dr. Fusella loves interacting with his patients, instilling trust in them, and growing the doctor-patient relationship. He believes considering the patient as a whole and taking this into account when making shared decisions is most important.

Dr. Fusella respects each patient by taking time to listen to them and enjoys developing a cooperative treatment plan that helps each patient become their healthiest. He believes there is no greater satisfaction than knowing he has positively impacted another life.

Dr. Fusella is originally from Niskayuna, NY, and decided to return to his hometown to practice medicine alongside his father and wife, both local family physicians. He enjoys spending time with his wife and children, traveling, playing sports, and gardening in his spare time.
