EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Lacey Weller

Biography

Lacey Weller is a board-certified family nurse practitioner. She received her Doctorate of Nursing Practice and Master of Science in nursing from SUNY Upstate Medical University in Syracuse, New York. Lacey earned her Bachelor of Science in neuroscience and Spanish from Union College in Schenectady, New York, and a Bachelor of Science in nursing from George Washington University in Ashburn, Virginia.

Her favorite part of working in primary care is seeing whole family units and watching everyone grow and succeed. Every season of life has its own special milestones, and Lacey loves being part of her patients’ and their families’ lives throughout those seasons.

Lacey is an avid runner and dancer and enjoys spending time outdoors with her son, husband, and rescue dog, Linus!
