Some of our patients have received letters in the mail from Change Healthcare regarding a data breach.
Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Kimberly Collen

Biography

Originally from New Paltz, NY, Kimberly Collen is a board-certified physician assistant. She completed her physician assistant program at Long Island University, and her bachelor’s degree in biology from Union College in Schenectady, NY. Her favorite part of being a physician assistant is meeting new people and helping solve their health issues.

Outside of her work, Kimberly enjoys hiking, sports, photography, and spending time with her family and two Labrador Retrievers. She is active in her community, serving as the Vice President of the Berlin Middle/High School PTO and volunteering at various school and sporting events.
