EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Hope Barnum

Biography

Dr. Hope Barnum is an active member of the American College of Osteopathic Family Physicians, the American Osteopathic Association, and the American Academy of Family Physicians. She received her Doctor of Osteopathic Medicine from the New York Institute of Technology College of Osteopathic Medicine in Old Westbury, New York. Dr. Barnum completed a family medicine residency at Nassau University Medical Center in East Meadow, New York. She began her studies at Alfred University in New York, where she earned her bachelor’s degree in biology. Dr. Barnum’s favorite aspect of family medicine and what she loves most about being a doctor is developing relationships with patients and providing care through all aspects and phases of the patient’s life.

In her spare time, Dr. Barnum enjoys singing, crocheting, and reading.
