Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Hayley Wynants

Biography

Hayley Wynants, a board-certified family nurse practitioner, is a seasoned healthcare professional with a Master of Science in nursing, family nurse practitioner from SUNY Polytechnic Institute in Utica, NY, and a Bachelor of Science in nursing from Russell Sage College in Troy, NY. With a rich experience at the bedside as a critical care pediatric RN since 2012, Hayley is well-equipped to provide comprehensive care to her young patients.

Hayley was born and raised in East Greenbush, NY. She loves her community and looks forward to making a difference in the lives of members of her hometown. Hayley was inspired by the compassionate care her grandmother received during her illness, which not only eased her grandmother’s suffering but also provided her family with immense support and comfort. This experience ignited her passion for healthcare and motivated her to pursue a career where she could offer the same care and empathy. Knowing firsthand how the compassion of others can leave a lasting impact, Hayley knows it would be a privilege to have the opportunity to leave that impact on patients within her community.

Hayley chose to work in the medical field because of her profound desire to make a tangible difference in people’s lives. Her passion was solidified after volunteering in the pediatric unit at a local hospital during high school. She was particularly moved by a situation where a multidisciplinary team worked tirelessly to improve the condition of a critically ill child with cancer. The combination of expertise, empathy, and teamwork saved the child’s life and provided immense support to the family. This experience made her realize the impactful role healthcare providers have, driving Hayley to pursue a career where she could contribute to such meaningful work.

Hayley believes communication and trust are the two most important building blocks to provide excellent patient care. As well as, developing rapport in a safe, non-judgmental environment allows patients to communicate freely, allowing for more patient-centered care. As a nurse practitioner, Hayley is a dedicated, hardworking professional who plans on taking a holistic approach to caring for her patients. Treating patients individually and catering treatment plans to each patient’s needs will allow Hayley’s patients to feel seen and understood. She plans on having open lines of communication with her patients while developing trusting, professional relationships across the lifespan.

Outside of medicine, she enjoys spending time with her family and friends. Hayley is a proud mother of two girls who take up much of her spare time. Their favorite pastime as a family is traveling to new places and exploring!
