EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Biography

Originally from Clifton Park, NY, Dr. Emma Smith is a resident physician at Albany Family Medicine in Albany, NY. As an undergrad, she attended SUNY Geneseo in Geneseo, NY, followed by SUNY Upstate Medical School in Syracuse, NY. Being able to serve the community she grew up in is one of the reasons she was drawn to family medicine, as it allows physicians like herself to be integral members of the community in which they are part. After being an avid sports player her whole life, Dr. Smith is interested in sports medicine, as well as adolescent medicine. Outside of medicine, she finds solace in reading non-medical books. She is a proud dog mom to Leo, a Havanese, and her partner shares their home with a Golden Retriever named Murphy. Exploring the outdoors and discovering new parks and areas in the Capital Region is a passion Dr. Smith indulges in during her free time. She also recently took up pickleball, a new hobby that she enjoys with her family.
