EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Donna Sgarlata

Biography

Certified Nurse Practitioner Donna Sgarlata earned her title as a Registered Nurse from Ellis Hospital School of Nursing. She went on to receive her Bachelor of Science from Empire State College. Donna became a certified family nurse practitioner at Community General Hospital in Syracuse; she then earned her master’s degree from SUNY Utica-Rome. Donna is a member of the American Academy of Nurse Practitioners.

Prior to joining Community Care Physicians, Donna spent 22 years in various disciplines, including emergency care, geriatrics, primary care, family medicine, college health, and women’s health. Donna chose to specialize in internal medicine because she wanted to be able to take care of patients over their lifespan. She also has a particular medical interest in geriatrics. Donna had always known she wanted to be in medicine since she was young; she naturally stepped forward as a caretaker whenever she got the chance. She is also the oldest of 5 children and was a natural nurturer. Her favorite part of her job is getting to hold the hand of a patient, seeing a glimmer in their eye, and knowing that she has made a connection with that patient.

In her spare time, Donna loves to read, garden, and listen to all kinds of music. She also likes golfing, traveling, enjoying nature, and volunteering at the Regional Food Bank.
