EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Biography

Originally from Northern California, Dr. Danielle Weismann is a resident physician at Albany Family Medicine in Albany, NY. She completed her undergraduate degree in mathematics at Bryn Mawr College in Bryn Mawr, PA, and a post-baccalaureate pre-health certificate at the University of Pennsylvania in Philadelphia, PA. Dr. Weismann spent several years in clinical research in the Philadelphia area before obtaining her medical degree at the Western University of Health Sciences College of Osteopathic Medicine of the Pacific Northwest in Pomona, CA. During medical school, Danielle’s research delved into patients’ perspectives on pain scales. She was drawn to family medicine by the opportunity to foster long-term patient relationships and provide a broad scope of care for individuals and communities. As she embarks on her family medicine training, she is excited to explore the Capital Region area and engage with the local community. Beyond her professional life, Danielle finds joy in spending time with her family, exploring the outdoors through hiking, and finding comfort in the art of baking.
