EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Cindy Chan

MDCM, FRCPC, FACP

Biography

Dr. Cindy Chan is a board-certified internist, fluent in French. She received her medical degree from McGill University in Montreal, Canada, and trained in internal medicine at the Montreal General Hospital of McGill University, where she served as Chief Resident during her senior year. In addition, Dr. Chan completed a 16-month fellowship in general internal medicine at the McGill University Health Center. Following her training, she worked in Framingham and Southboro, Massachusetts, where she helped train internal medicine residents. She is a Fellow of the American College of Physicians, a Diplomate of the American Board of Internal Medicine, a Fellow of the Royal College of Physicians of Canada, a Specialist of the College des Medecins du Quebec, and a Licentiate of the Medical Council of Canada. Dr. Chan is also the Chief of the Department of Internal Medicine at Sunnyview Rehabilitation Hospital, acting on their Medical Executive Committee.

Dr. Chan loves the long-term relationships she establishes with her patients and their families. She believes seeing each patient as a person and not as a collection of health problems is vital for quality care.

Each year, Dr. Chan helps organize the annual Stick It To Brain Tumor Tournament, raising funds for the Pediatric Brain Tumor Foundation. They have raised over $200,000 in 10 years! In her spare time, she enjoys hiking, hockey, skiing, baking, and cake decorating.
