Some of our patients have received letters in the mail from Change Healthcare regarding a data breach.
Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Dr. Mehta received her doctorate from the New York College of Osteopathic Medicine. She completed her residency at North Shore University Hospital, part of NYU’s Medical College. She is board certified in pediatrics and has a particular interest in caring for children with disabilities.
