EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

April St. Peter

Biography

April St. Peter is a board-certified family nurse practitioner. She earned her Master of Science in nursing – family nurse practitioner from Sage Graduate School. April earned her Bachelor of Science in biology from Siena College and Bachelor of Science in nursing from Russell Sage College. April is also a member of the Nurse Practitioner Association of New York State and the American Nursing Academy. Prior to joining Community Care Physicians, April spent four years working as a NICU nurse. It was her experience as a NICU nurse that revealed her passion for creating relationships with families and advocating for vulnerable patients.

In her free time, April enjoys spending time with her husband, Ryan, and their daughter, Rylen. She is a huge advocate for the March of Dimes and is grateful for the relationships she has formed over the years.
