Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Anita Puthuparampil-Suchdeve

Biography

Board-certified family nurse practitioner Anita Puthuparampil-Sucheve, FNP-BC, earned her master’s degree from Sage College as a family nurse practitioner. Anita has spent over 15 years working in the vascular specialty and has been a part of Vascular Health Partners since its beginning. She enjoys working with vascular patients and is thrilled to take her love for vascular to the next level by providing quality care as a nurse practitioner. When caring for patients, she believes educating patients about their vascular disease is most important.

Anita is an active member of the Society for Vascular Nursing and the current president of the Northeast Society for Vascular Nursing. She has had the opportunity to be a presenter at both the Society for Vascular Nursing and the Center for Vascular Awareness.

In her spare time, Anita enjoys spending time with her family and being a mother to 2 teenagers. She enjoys volunteering in different community outreach programs.
